Hospice care is family focused, physician directed care for those with life-threatening illnesses and their loved ones. Hope Hospice, Inc. specializes in physical, emotional, spiritual and psychosocial care for persons with a prognosis of six months or less to live.

Hospice care neither prolongs life nor hastens death. Our specialized team of health care professionals and volunteers provide comfort and support to patients and families when a life-limiting illness no longer responds to cure-oriented treatments. Hospice staff continually strives to offer the highest level of skill and knowledge on end-of-life issues.

The Hospice Team

Tailored to fit each individual's needs, our hospice team of professionals and volunteers provide services wherever a person calls home. We focus on providing personalized comfort care that enables a person to remain in his/her home, live as independently as possible and be surrounded in love. The hospice team includes the following disciplines:

  • Primary Physician: The primary physician provides oversight of the hospice patient by continually communicating with hospice staff and providing their medical recommendations.
  • Hospice Medical Director: The hospice medical director is also available to your primary care physician and the hospice team for medical input and advice. At times, the medical director will also manage certain aspects of the care, in cooperation with the primary physician and the patient and family. The medical director may be utilized when necessary if the primary physician cannot be reached.
  • Hospice Nurse: The hospice nurse provides expert pain management and symptom control techniques. The nurse keeps the primary physician and family informed of the patient’s condition on an ongoing basis. The Nurse provides the complete spectrum of skilled nursing care and is available 24 hours a day, seven days a week.
  • Hospice Home Health Aides: The hospice home health aide meets all personal needs of the patient such as bathing and grooming. Home health aide services are under the supervision of the nurse case manager with a written plan of care to ensure all patients’ needs are being met.
  • Medical Social Worker: The hospice social worker provides assistance with any financial or psychosocial concerns. The social worker can also aid those needing funeral arrangements, nursing home placement or additional resources that can be obtained in the community.
  • Chaplain: The hospice chaplain provides spiritual support and guidance to patients and families, often serving as a liaison between them and their religious community.
  • Massage Therapist: The massage therapist uses different massage techniques to feed and soothe the body and spirit. Massage may be a way to manage pain, improve poor circulation or simply a way to reduce anxiety. The massage sessions are modified to address specific symptoms and achieve maximum results.
  • Bereavement Counselor: The bereavement counselor provides support services and resources to the families of deceased patients. Grief counseling is also provided prior to a patient’s death as people may begin to grieve as early as at the time of learning of a terminal illness.
  • Trained Non-Medical Volunteers: The hospice volunteer provides companionship and comfort to those suffering from the effects of a terminal illness.
